The Virginia Crockford Health Planning Award was established at the Central Virginia Health Planning Agency’s (CVHPA) 25th Anniversary celebration, held in 2001, to honor the memory of Virginia Crockford. A long time member of the CVHPA’s Board of Directors and Capital Area Health Advisory Council, Ms. Crockford was involved in health care planning since its infancy in Virginia. She served as Richmond City’s School Board Chairman during the period of desegregation and was an active community volunteer for numerous health and educational issues. Therefore, this award is designed to recognize outstanding advocacy of and leadership in promoting health planning and community health improvement.
The CVHPA is seeking nominees for this year’s award, which will be presented at the CHVPA’s Annual Board of Directors meeting on October 1, 2008. A committee composed of current CVHPA Board members will review the nominations received based on the established criteria and select the recipient. Current Board members are excluded from consideration.
The following criteria will be used to select the award recipient:
· Health Planning and Community Improvement Service to Central Virginia (Health Planning Region IV) – include specific service/activities outside of the person’s current occupational responsibilities
· Volunteer Service (for example, human and health related organization affiliations, other related information that shows the person’s involvement in improving the community’s quality of life)
· Considerable Dedication to Health Planning Principles (for example, health status improvement through access to quality and cost effective human services)
Please submit the completed nomination form, providing specific examples for each criterion, and include the nominee’s biographical sketch, resume, or curriculum vitae if available. Please return the form and supporting information to our office (via fax or mail) by 5 p.m. on Friday, August 15, 2008.
